Deductibles and co-pays are due the day of the scheduled procedure.

 Insurance and your Bill

  1. Prior to surgery at GulfCoast Surgery Center, Inc., you will be contacted regarding your insurance coverage and any deductibles or co-pays for which you are responsible. If you have not been contacted about your insurance coverage before your scheduled procedure, please call GulfCoast Surgery Center, Inc. at (941) 924-9282.  If you have questions regarding coverage or pre-approval requirements, please contact your insurance company.

  2. As a courtesy, we will file your insurance claim for you. We will ask you to sign an "Assignment of Benefits" form so that the payment comes directly to GulfCoast Surgery Center, Inc. Any amount not paid by your insurance, such as deductible and co-insurance, will be collected on the day of surgery unless prior arrangements have been made,

  3. GulfCoast Surgery Center Inc.'s bill does not include any services rendered by your physician, the anesthesiologist, or a laboratory or any pathology services.
